Online Triage

eConsult allows you to quickly and safely get help and advice on non urgent and administrative matters from your own doctors and GP practice online, for free, from anywhere.

eConsult is a form-based online consultation & triage platform the collects your medical or administrative request and sends it through to your GP practice to triage and decide on the right care for you and everyone else.

Urgent Appointment

To request an urgent appointment (Monday to Friday):

  • phone us on 01429 222170, Monday to Friday from 8am to 6pm

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

 

 

Routine Appointment

To request a routine appointment in the next 7 days:

Care Navigation

Our receptionists, who are our care navigators, will not automatically make you an appointment.

Instead, you will be asked a series of questions to ensure you are seen by the most appropriate healthcare professional.

Healthcare professionals may include: GPs, Nurse Practitioners, Nurses, Community Pharmacists, Physiotherapists, Mental Health Workers, Social Prescribers, etc.

You may also be signposted to other available services. Your cooperation with the reception team whilst they care navigate would be greatly appreciated.

Coming along to the practice does not guarantee an appointment and does not give patients priority. All appointment requests will be care navigated whether patients attend the practice or telephone. Thank you for your cooperation

Nurse Appointments

All nurse clinics are by appointment only and operate every morning and afternoon, Monday to Friday. We kindly ask that patients try to arrive on time for their appointment. Patients who arrive late for their allocated appointment, may not be seen and will be asked to make a new appointment. If you suspect you may have covid symtoms, please do a test before attending the surgery.

Home visits

Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception. 

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls

You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

7 day extended access

Hartlepool & Stockton Health are offering 7-day access to GPs, Nurses, Healthcare Assistants, and Physiotherapists at the following locations:

  • Chadwick Practice, Hartlepool
  • Tennant Street Medical Centre, Stockton
  • Woodbridge Practice, Ingleby Barwick

Appointments are available from 6:30pm to 8:00pm Monday to Friday, 10am to 5pm on Saturdays, and 11am to 4pm on Sundays. Please call your usual GP practice to book an appointment.

Your appointment

However you choose to contact us, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

Cancelling or changing an appointment

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

To cancel your appointment:

  • use your NHS account (through the NHS website or NHS App)
  • using the GP online system - SystmOnline
  • phone us on 01429 222170, Monday to Friday from 8am to 6pm
  • reply CANCEL to your appointment reminder text message

If you need help when we are closed

If you need medical help now, use NHS 111 online or Call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need a chaperone
  • if you need an interpreter
  • if you have any other access or communication needs
